Grafton Medical Partners are looking for a dynamic, collaborative qualified Pharmacist to join our practice. Located in Tooting South London, we serve a shared population size of around 45,000 patients. Our ideal candidate would be able to work in Tooting, South London on a part-time or full-time basis. We are open to discussing working patterns to fit both your needs and the Practices.

Main duties of the job

  • Support the Partners in implementing effective medicine management and patient care within the practice/s.
  • Identify areas for improvement and managing change.

Grafton Medical Partners is a busy, forward-thinking PCN made up of Trevelyan House Surgery, MacMillan Way Surgery and Upper Tooting Surgery. We value staff development, training, and excellent patient care and have a long-established training practice at Trevelyan House.

Job responsibilities

  • Deliver patient-facing clinical care: Conduct consultations, prescribe medication, run clinics (especially for long-term conditions), and support vulnerable or complex patients, including home visits.
  • Enhance patient understanding and outcomes: Educate patients and carers on medication use, improve adherence, and provide ongoing support to build confidence in treatments.
  • Ensure safety and quality of prescribing: Monitor high-risk drugs, handle medication incidents, carry out audits, and align practice with clinical standards (e.g., NICE) to improve safety and performance.
  • Collaborate, train, and develop services: Work within the multidisciplinary team, support staff training, liaise with external stakeholders (e.g., community pharmacists), and contribute to service improvement and professional development.
  • Improve medicines management systems: Oversee prescribing processes, medication reviews, repeat prescriptions, and ensure safe, effective, and high-quality use of medicines across the practice.

Person Specification

  • Experience: Relevant post-registration experience of community, hospital and/or primary care pharmacy practice.
  • Qualifications: Post-graduate qualification to Diploma or Masters level in a recognised area of pharmacy practice or equivalent.
  • A working understanding of NHS policies, processes, structures, systems and clinical practices.
  • Knowledge of the Community Pharmacy Contract.
