The predicted salary is between 40735 - 43516 £ per year.
We are looking for social workers to join our North Transfer of Care Team, working out of Hinchingbrooke Hospital, as well as covering Kings Lynn Hospital and Peterborough City Hospital on a rota basis.
We provide support to adults being discharged from hospitals and rehabilitation settings. You will be imaginative and dynamic and make a positive contribution to delivering good outcomes for people who have care and support needs. You will be a person-centred practitioner with excellent communication skills and be able to work within a multidisciplinary setting. The Transfer of Care Teams operate within specific office hours, but that does not mean we will not consider flexible working opportunities. Our staff receive a salary enhancement between 7% and 15% depending on their selected working pattern:
- 7% for 1 weekend day a month and 1 Bank Holiday a year
- 8% for 1 weekend day a month and 2 Bank Holidays a year
- 10% for 1 weekend day a month and 3 Bank Holidays a year
- 15% for 2 weekend days a month and 3 Bank Holidays a year
You will still only work 5 days per week (full-time), meaning you get a day off during the week that you’re working at the weekend or Bank Holiday. While this is a fast-paced and highly demanding job, our team offers excellent support from both colleagues and managers, and opportunities to develop your knowledge. We are able to balance the work that needs to be done and what we can do to support you to keep on top of your workload.
The role will work across 3 Acute Hospitals: Addenbrookes (Cambridge), Hinchingbrooke (Huntingdon) and Peterborough City Hospital. We also support 5 Inpatient Rehabilitation centres across Cambridgeshire and Peterborough. We know that your time is precious, so our interviews are generally held virtually, by Microsoft Teams, to provide the most flexible recruitment process. We may include a virtual assessment as part of this, but we’ll give you all of the relevant information ahead of time.
You will be responsible for completing Care Act assessments, Support plans, Mental Capacity assessments, and other statutory workflow tasks to support individuals in their safe and timely discharge from hospital settings. You will also complete initial community reviews to ensure that the discharge support is meeting the needs of the individual.
We are looking for confident and dynamic individuals who enjoy making a difference in their local community, are curious and spot things that others might not see, enjoy building relationships and like to explore possibilities.
You must:
- Be a qualified Social Worker with a Social Work England registration
- Have the ability to work from a variety of locations and travel countywide independently
