The Practice Nurse / Chronic Disease Nurse will work as part of the multidisciplinary team across Greenhill Medical Centre & Clase Surgery, reporting to the GP Partners and Practice Manager. The role will involve weekly chronic disease clinics, including diabetes, asthma, COPD, hypertension and cardiovascular disease, alongside routine practice nursing duties such as health checks, vaccinations, wound care, ECGs, observations and health promotion.

The post-holder will maintain accurate clinical records, support patient recalls, contribute to audits and quality improvement, and work closely with GPs and the wider practice team to provide safe, high-quality patient-centred care. Training and development opportunities will be supported where required, subject to the candidate's experience and role requirements.
