We are seeking a motivated and compassionate Home Visiting Practitioner to join our primary care team. The successful candidate will provide same-day home visits to patients who are housebound or too unwell to attend the surgery. Working closely with our GPs, nurses, and wider multidisciplinary team, you will assess patients in their homes, provide clinical management where appropriate, and help ensure patients receive the right care at the right time. This role plays an important part in supporting patient care while improving GP capacity within the practice.

Responsibilities

  • Conduct home visits for patients who are unable to attend the surgery.
  • Undertake clinical assessment, examination, and management of patients in their homes.
  • Develop and implement management plans, including prescribing where qualified.
  • Provide safety-netting advice and arrange follow-up care where required.
  • Liaise with GPs, district nurses, community teams, and secondary care services.
  • Maintain accurate and timely clinical records using the practice clinical system.
  • Identify patients requiring urgent escalation or hospital admission.

We are one of the largest GP practices in Wales serving over 25,500 patients across three sites. We have a great team of 7 partners, 10 salaried GPs, 6 nurses, 4 HCAs and a clinical pharmacist. We have first contact physio every day.
