Trosnant Lodge Medical Practice is looking for a clinical pharmacist to join our team. The successful candidate will play a key role in delivering high quality pharmaceutical care, supporting safe and effective prescribing and improving patient outcomes across a range of clinical services. The pharmacist will work collaboratively with the clinical and administrative teams to assess patients, manage long term conditions, conduct medication reviews and help with the management of repeat prescribing. They will also be expected to contribute to audits, quality improvement projects and any other prescribing incentives. The role requires strong clinical knowledge, excellent communication skills and a commitment to delivering patient centred care. Due to room availability in the practice, the hours for this role will be predominately afternoons.
Main duties of the job
- Improve the quality and effectiveness of medicines management in the practice.
- Perform structured medication reviews and medicines optimisation.
- Provide patient focused healthcare.
- Support the management of chronic diseases such as hypertension, diabetes, CKD, asthma and COPD.
- Provide advice and education to patients regarding medicines and treatment plans.
- Maintain accurate clinical records and ensure compliance with professional standards.
- Contribute to service development and quality improvement initiatives.
- Assist with the management of repeat prescribing, including medication authorisation and dealing with requests for acute medications.
- Medicines management tasks such as adding new medications and removing discontinued medications following secondary care reviews.
- Clinics will be face to face, with the possibility of some telephone reviews.

We are a friendly, well-established practice based in Pontypool, South Wales. We aim to offer high quality clinical care to out patients while achieving a good work/life balance. Our team is comprised of 3 GP partners, 2 Salaried GPs, an Advanced Nurse Practitioner, 3 experienced practice nurses and 2 phlebotomists, alongside good administrative support.
Job responsibilities
- Ensure compliance with NHS guidelines, local policies and professional standards.
- Develop and embed safe, efficient prescribing processes within the practice.
- Contribute to clinical audits, quality improvement initiatives and service development.
- Support safer prescribing of high risk medications e.g. DMARDS, anticoagulants, opioids.
- Conduct medication reviews.
- Prescribe medications safely and appropriately as an independent prescriber.
- Support the management of long-term conditions such as diabetes, hypertension, CKD, asthma, COPD and CVD.
- Review and optimise patient medications to improve safety, adherence and clinical outcomes.
- Design, implement and refine protocols, SOPs and medicine workflows.
- Identify and reduce prescribing risk, polypharmacy issues and medication errors.
- Provide patients with advice and education regarding medicines and treatment plans.
- Maintain accurate and up to date clinical records.
- Participate in continuing professional development and mandatory training.
- Authorising repeat and acute medications.
- Dealing with patient medicine queries.
- Adding and removing medications at the request of secondary care.
Person Specification
- Registered pharmacist with the General Pharmaceutical Council (GPhC).
- Masters degree in Pharmacy (MPharm) or equivalent recognised pharmacy qualification.
- Post registration experience as a pharmacist.
- Experience undertaking medication reviews and medicines optimisation.
- Knowledge of prescribing guidelines, clinical governance and medicines management.
- Experience working as part of a multidisciplinary team.
- Excellent communication and patient consultation skills.
- Experience using electronic prescribing and clinical record systems.
- Previous experience working within a GP practice.
- Experience managing long term conditions.
- Experience conducting structured medication reviews.
- Experience supporting quality improvement, prescribing audits and medicines safety initiatives.
- Familiarity with Welsh NHS systems and prescribing policies.
