Princess Street Surgery is a friendly and patient-focused GP practice committed to delivering high-quality primary care services to a diverse patient population. The Practice Nurse will work as part of a multidisciplinary team, providing evidence-based nursing care, health promotion, chronic disease management, and preventative healthcare services. The post holder will work collaboratively with GPs, Nurse Practitioners, Healthcare Assistants, Pharmacists, and the wider practice team to ensure safe, effective, and compassionate patient care. Job summary

Main duties of the job

  • Chronic Disease Management – Support patients in self-management and lifestyle modification; monitor and review patients in accordance with local and national clinical guidelines; maintain chronic disease registers and implement effective call/recall systems.
  • Immunisations and Vaccinations – Administer adult and childhood immunisations; undertake Baby Clinic sessions; conduct travel health assessments; provide appropriate travel vaccinations and advice; ensure safe storage, handling, stock control and cold chain maintenance of vaccines and medications.
  • Women’s Health and Family Planning – Provide family planning and contraceptive advice; perform cervical cytology screening following national guidelines.
  • Wound Care and Clinical Procedures – Assess and manage acute and chronic wounds, provide dressings and ongoing management; monitor patients receiving anticoagulation therapy; support other clinical procedures as required.
  • Health Promotion – Deliver health promotion and lifestyle advice on smoking cessation, healthy eating, weight management, alcohol consumption and physical activity; support public health initiatives and national health campaigns.
  • Support the Infection Prevention Control (IPC) lead and IPC policy implementation.
  • Consult with patients and families to assess needs and undertake care planning.
  • Assist with quality improvement and assurance initiatives within the organisation.
  • Develop, implement and embed an effective care management programme for frail and housebound patients.
  • Embed population health management approaches targeting health inequalities.
  • Provide routine nursing care following clinical evidence, NICE and the NHS Services Framework.
  • Support new-to-practice nurses and other healthcare professionals.
  • Participate in clinical audits, research projects and protocols development.
  • Recognise professional boundaries and refer patients appropriately.
  • Support enhanced services and other service requirements.
  • Ensure compliance with safeguarding, safeguarding adults and children legislation.
  • Participate in patient complaints management and incident learning.
  • Complete mandatory training and induction programmes.
  • Maintain accurate clinical records with appropriate coding.
  • Lead on public health campaigns such as flu clinics and travel advice services.
  • Maintain a clean and effective working area.
  • Coordinate the nursing team rota and support staffing planning.

Person Specification – Qualifications

  • Essential – BSc (Hons) in Nursing or an approved diploma and active registration with the Nursing and Midwifery Council (NMC).
  • Desirable – Chronic Disease Diplomas; Women’s Health Certificate.

  • Essential – Willingness to learn and acquire new skills; understanding of current issues in nursing and the NHS; appropriate knowledge base and audit understanding; excellent computer skills; health and safety knowledge.
  • Desirable – Experience of working in primary care.
