Main duties of the job
The post-holder will be expected to undertake all the normal duties and responsibilities associated with a GP working within Primary Care.
About us
We are a large dual site medical group covering 25,000 patients in the lower Swansea Valley. We have a strong and stable multi-disciplinary team consisting 14 GPs, 2 advanced nurse practitioners, a paramedic practitioner, 2 clinical pharmacists and a 1st contact advanced physiotherapist. They are supported by our team of 10 nurses and 2 HCSW.
As with any organisation we are looking for enthusiastic reliable staff to come join the team be it on a short or long term basis. We'd be very interested to hear from you. We have educational meeting every fortnight ensuring we develop and help one another. There is ample opportunity to develop a sub specialty - clinical or non clinical.

Job Summary
The post holder will provide high quality, safe and effective medical care to patients registered with Cwmtawe Medical Group.
The Salaried GP will-

• Provide the full range of general medical services appropriate to General Practice.
• Assess, diagnose, treat and manage patients presenting with acute, chronic and undifferentiated conditions.
• Work autonomously within their professional competence and in accordance with GMC standards.
• Work collaboratively with GP Partners, other GPs and members of the wider Multidisciplinary Team to meet the needs of patients.
• Contribute to the effective delivery and ongoing development of services within the practice.
• Work in accordance with practice policies and procedures and relevant NHS Wales requirements.

Key Responsibilities
Clinical Responsibilities

• Undertake consultations with patients presenting with acute, chronic and undifferentiated conditions.
• Assess, investigate, diagnose and manage patients appropriately.
• Develop appropriate treatment and management plans in partnership with patients.
• Prescribe medication safely and appropriately in accordance with evidence-based practice, national guidance and practice protocols.
• Review medication for effectiveness, safety and ongoing clinical need.
• Arrange and interpret appropriate investigations.
• Make referrals to secondary care and other health and social care services where clinically appropriate.
• Provide appropriate health promotion, disease prevention and lifestyle advice.
• Provide appropriate management and monitoring of patients with long-term conditions.
• Recognise and respond appropriately to urgent and emergency presentations.
• Undertake home visits where clinically appropriate and in accordance with practice arrangements.
• Provide telephone, digital and face-to-face consultations in accordance with the practice model of care.
• Provide appropriate follow-up and continuity of care.
• Maintain awareness of and act appropriately upon safeguarding concerns relating to children and adults.

Clinical Governance

• Work within practice and local policies and procedures.
• Participate in clinical governance activities within the practice.
• Participate in significant event reviews, clinical audit and quality improvement activity as required.
• Contribute to the development and implementation of practice clinical policies, protocols and pathways.
• Support the practice in delivering relevant contractual and quality requirements.
• Participate appropriately in the investigation and learning arising from complaints, incidents and patient safety events.
• Maintain appropriate standards of infection prevention and control.
• Use NHS and practice resources appropriately and responsibly.

Team Working

• Work as an effective and responsible member of the practice team.
• Work collaboratively with GP Partners, Salaried GPs, GP Registrars, Nursing staff, Advanced Practitioners, Allied Health Professionals and other members of the Multidisciplinary Team.
• Communicate clearly and effectively with clinical, management, reception and administrative teams.
• Recognise the roles and skills of other members of the team and refer or delegate appropriately.
• Support colleagues and contribute positively to the development of effective ways of working.
• Participate in practice and clinical meetings as required.
• Establish and maintain effective working relationships with external healthcare providers, secondary care and other relevant agencies.
• Work across practice sites where required in accordance with practice arrangements.

Administration and Professional Responsibilities

• Participate in the administrative and professional responsibilities of the practice team.
• Ensure accurate, clear and contemporaneous records of all consultations and clinical activity are maintained.
• Ensure clinical computer systems are kept up to date and information is recorded accurately.
• Review and act upon pathology results, correspondence, hospital letters and other clinical information in a timely manner.
• Complete referrals, reports and other clinical documentation appropriately.
• Complete NHS documentation and certification appropriate to the role.
• Attend and participate in practice meetings as required.
• Contribute to the effective and efficient running of the practice.

Training and Personal Development

• Maintain full registration and a licence to practise with the General Medical Council.
• Maintain appropriate medical indemnity arrangements.
• Participate in annual appraisal and meet the requirements for GMC revalidation.
• Take responsibility for maintaining continuing professional development.
• Maintain a Personal Development Plan.
• Ensure mandatory training is completed and maintained in accordance with practice requirements.
• Remain up to date with developments in General Practice, clinical guidance and relevant NHS Wales policy.
• Participate in educational meetings, courses and study as appropriate to professional development.
• Support the education and development of colleagues, GP Registrars, medical students and other learners where appropriate.

• Ensure patients are appropriately informed and involved in decisions regarding their care.
