Oak Tree Surgery is seeking a motivated and compassionate Practice Nurse to join our friendly primary care team. This role involves delivering high-quality nursing care within a busy GP practice, including chronic disease management, immunisations, cervical screening, wound care, and health promotion.
You will work collaboratively with GPs, healthcare assistants, and administrative staff to support patient wellbeing and preventative care, while contributing to quality improvement within the practice.

Job responsibilities
The Practice Nurse is responsible for delivering high-quality, evidence-based nursing care to patients registered at Oak Tree Surgery. Working as part of a multidisciplinary primary care team, the post-holder will provide holistic care, support long-term condition management, promote health and wellbeing, and contribute to quality improvement initiatives within the practice.
Clinical Duties
- Deliver chronic disease management clinics (e.g., diabetes, asthma, COPD, hypertension) – training provided
- Administer childhood and adult immunisations in accordance with national guidance
- Undertake cervical cytology screening
- Provide wound care management, including dressings and suture removal
- Carry out health checks and other preventative health screenings
- Perform travel health assessments and vaccinations (where trained)
- Provide lifestyle advice and health promotion interventions
- Recognise and respond appropriately to safeguarding concerns
- Maintain accurate, contemporaneous patient records in line with professional standards
- Assist in the delivery of research projects – training provided
Patient Care & Communication
- Deliver patient-centred care that respects dignity, diversity, and confidentiality
- Provide clear health education and self-management support
- Communicate effectively with patients, carers, and the wider healthcare team
- Support patients in shared decision-making about their care
Quality & Governance
- Contribute to QI projects and enhanced service delivery
- Participate in clinical audit and quality improvement initiatives

Team Working
- Work collaboratively with GPs, healthcare assistants, pharmacists, and administrative staff

Person Specification
- Registered Nurse with current NMC registration
- Competence in immunisations and cervical cytology (or willingness to train)
- Experience in long-term condition management (or willingness to train)
