GP Direct is seeking an experienced Care Coordinator to join our well-established and dynamic team. You will work closely with the admin team and clinicians to provide responsive and efficient care for our patients, coordinating patient care, patient recalls for QOF and Enhanced Services, supporting clinicians with relaying and managing blood results follow up, document management and assisting with prescription queries. This role is ideal for an experienced practice administrator who has an intimate knowledge of QOF, Enhanced services and all matters related to clinical coding and searches & reporting in EMIS Web.
The Care Coordinator will provide dedicated administrative support to clinicians, ensuring safe and efficient management of patient care processes. A key aspect of the role will involve coordinating and following up on clinical results including blood tests, ensuring that all actions are completed accurately and in a timely manner. The post-holder will support the management of prescription queries, liaising with clinicians, pharmacies, and patients to resolve issues and ensure continuity of care. The role will include supporting with clinical correspondence management, such as follow up on actions for the practice and liaising with patients and clinicians. They will play an active role in delivering QOF and Enhanced Services requirements by coordinating patient recalls, maintaining accurate records, and supporting achievement of practice targets.
In addition, the Care Coordinator will be responsible for clinical coding, running searches, and producing reports using EMIS Web to support population health management and performance monitoring. The role requires a strong understanding of GP systems, attention to detail, and the ability to prioritise workload effectively while maintaining high standards of data quality and patient care.

The following are the core responsibilities of the Care Coordinator. There may be, on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:
- Assist clinicians with relaying and managing actions following blood results, this may be via phone and SMS. This will likely relate to providing a one-stop recall process, so patients reviews can be rationalised to avoid unnecessary appointments and duplication.
- Coordinate recall process for long-term conditions and reviews, ensuring all aspects of QOF and Enhanced Services are taken into account.
