The primary role is to support the delivery of a high-quality Reablement Service within the Intermediate Care Centre. This service aims to prevent unnecessary hospital and care home admissions while supporting early discharge from hospital. This is a rewarding opportunity to make a real difference by supporting people to remain independent and avoid unnecessary hospital stays. You will play a key role in helping individuals rebuild their confidence and live as independently as possible.
Responsibilities
- You will work with individuals to help them regain confidence, independence, and daily living skills.
- You will support the delivery of a person-centred reablement service.
- Assist individuals to regain or maintain independence in daily living activities.
- Work collaboratively as part of a multidisciplinary team.
- Monitor and support individuals with complex health needs.
- Record and report baseline observations (e.g. vital signs).
- Assist with the supervised management of health conditions.
- Contribute to safe discharge planning and ongoing community support.
- Promote dignity, independence, and wellbeing at all times.
Qualifications
- You will preferably be educated to Level 3 standard (or equivalent).
- Have previous experience in a care setting.
- Have experience with administering medication.
- Have ability to work as part of a team and independently.
- Demonstrate good communication and interpersonal skills.
- Require a compassionate, patient-centred approach.
This role operates on a shift pattern, including unsocial hours. Enhanced rates of pay are provided to reflect the working hours.
