Overview
The post holder will exercise a high degree of personal and professional autonomy and have the ability to plan and reach complex and critical judgments and have decision making skills. The role involves delivering high quality clinical care within the Integrated Healthcare Service for the prison, encompassing acute & primary care, long‑term condition management, enhanced assessment and diagnostic intervention services, and health promotion activities, in line with the NHS Plan, public health indicators and National Service Frameworks.
Responsibilities
- Demonstrate robust expert clinical skills and a sound understanding of evidence‑based nursing practice to provide a proactive approach ensuring quality and outcome‑driven practice on a day‑to‑day basis.
- Support the development of evidence‑based practice in the specialist field and promote research as appropriate.
- Participate in managerial and professional clinical supervision programmes, training, and annual appraisal, ensuring it becomes an integral part of team practice.
- Ensure implementation of effective and appropriate patient‑centred care planning tools, long‑term condition registers, and consistent delivery across all prisons within the service.
- Work collaboratively with the Clinical Lead, GPs and other clinicians to meet and review service needs as required.
- Form part of skilled multidisciplinary teams comprising all healthcare staff, including strategic partners, and prison management to deliver a single referral pathway.
- Undergo training to maximise uptake of screening, health promotion and prevention activity, including vaccinations.
- Ensure a named care co‑ordinator is allocated for every patient with complex needs, using a proactive, evidence‑based approach to clinical interventions, health promotion/prevention activity and management of acute and long‑term conditions via our stepped‑care approach.
Qualifications and Application Requirements
- All applicants must be willing to undertake National Security Vetting as part of pre‑employment checks through Oxleas and the prison vetting team.
- Proof of right to work documentation.
- Proof of ID (1 photographic ID).
- Proof of address documentation.
- Non‑UK passport holders must provide correct right‑to‑work documentation and a Home Office Share code.
- Address history: 5 years of UK address history required. If less than 5 years, a Police Certificate in English from the previous country of residence is required.
- UK passport holders who have lived abroad for more than six months in the last three years must provide a certificate of good conduct or an overseas police check in English from the relevant countries.
