An exciting opportunity has arisen to join the Community Reablement Team for the reablement and support of people within Redcar and Cleveland area. Join us in providing essential, rapid response care within the comfort of a person's home for up to seven days. The rapid response service makes a real impact in people's lives preventing hospital and care home admissions alongside supporting hospital discharges. The team also supports people in their own homes for up to 6 weeks helping them to regain their ability and confidence to live independently. The rota is a two‐week pattern of four days a week and four time periods in the day:
- Week one: Monday, Tuesday, Thursday, Friday.
- Week Two: Wednesday, Thursday, Saturday, Sunday.
Hours are worked in three or four separate time periods morning, lunch, tea and evening: 3hrs, 2hrs, 2hrs, 2.5/3 hrs apart from Thursdays. The successful candidate will have the ability to meet the travel requirements of the post and have the use of a vehicle.
