Summary
The post holder will:
- Provide Physiotherapy assessments and interventions to individuals requiring Physiotherapy within the Complex Care at Home service.
- Undertake all aspects of clinical duties as an autonomous practitioner with support and guidance from the Physiotherapy Professional Lead and Clinical Specialists Physiotherapists within the trust.
- Provide advice and support for all members of the MDT involved in the patient's care. For example, Matrons and Wellbeing Coordinators within the CC@H service.
- Evaluate progress, re-assess and amend exercise and treatment programmes as required using specialist knowledge. To refer to other specialist services as required.
