Our social workers play a vital role in supporting adults with learning disabilities to live safe, independent, and meaningful lives. In this role, you'll manage a varied caseload, undertaking holistic assessment and develop strengths-based care and support plans that reflect individual needs, preferences, and goals. You'll work closely with families, carers, and approved providers to coordinate effective, joined up support, identify and manage risk, in order to achieve mutually agreed outcomes.
