We’re looking for a Care Coordinator/Carer to join our brilliant team in Reading. This crucial role ensures our clients receive exceptional care.

The role involves:

  • Organising schedules based on clients' requested call times and care professional availability
  • Maintaining effective communication with staff, clients, families, and professionals
  • Delivering care directly

This position offers an excellent opportunity for professional growth and development within our expanding business, which is committed to providing superior home care. This role is currently a 50% service based role, you would be expected to cover care calls for half the week (M-F) and 50% of the role is care coordination.

Responsibilities:

  • Producing weekly rotas and care scheduling to ensure adequate provision of care
  • Undertake regular care visits to support our clients (all private clients)
  • Daily troubleshooting and problem solving
  • Managing staff absence, allocating extra calls, and cancelling care calls when necessary
  • Assisting in the training and induction of new team members
  • Facilitating Good Oaks in providing a continuous and reliable service

Qualifications:

  • You’re a highly organised and efficient co-ordinator with a people focused approach
  • You’re a great carer or supervisor looking to grow your skills and career
  • You’re a strong communicator with all types of people and ability to remain positive
  • You are IT confident enabling effective scheduling, team contact and work planning
  • You’re highly reliable and able to foster a strong team ethos and keep morale high
  • You are growth focused and support continued development
  • You are dedicated with high energy levels
  • You’re willing to undertake a DBS check to work with vulnerable adults
