We’re looking for a Care Coordinator to join our brilliant team in Aylesbury. This crucial role ensures our clients receive exceptional care.

The role involves organising schedules based on clients' requested call times and care professional availability, as well as maintaining effective communication with staff, clients, families, and professionals. This position offers an excellent opportunity for professional growth and development within our expanding business, which is committed to providing superior home care throughout Aylesbury. This role is currently a 90% service based role, you would be expected to cover care calls as required.

What will you do?

  • Producing weekly rotas and care scheduling to ensure adequate provision of care
  • Undertake regular care visits including being available evening and weekends when needed
  • Daily troubleshooting and problem solving
  • Managing staff absence, allocating calls, and cancelling care calls when necessary
  • Assisting in the training and induction of new team members
  • Facilitating GoodOaks in providing a continuous and reliable service
