Leicestershire County Council is recruiting a Reablement Team Leader (Charnwood locality) to supervise the Home Care Assessment Reablement team in community and hospital settings. The role includes first visits, risk assessments and guiding a team to deliver reablement plans.
You will work with Occupational Therapists and Physiotherapists to help service users regain independence, through goal-based social care tasks, using an asset-based approach.
