The predicted salary is between 50000 - 70000 £ per year.

Minimum of 4 sessions per week (x3 roles available) - fixed term contract until 31.3.2026 with a potential to be extended subject to national DES contracting. Working across PCN sites (maximum of x2 sites per role). Primary Care Networks (PCNs) are a key part of the NHS long-term plan. They provide the structure and funding for services to be developed locally in response to the needs of the patients they serve. There are three GP practices which make up Pioneers Integrated Partnership PCN. Following the release of ARRS GP information and funding we are looking for newly qualified (within 2 years and not having had a substantive post), enthusiastic, forward-thinking GPs to join Pioneers Integrated Partnership (PIP) PCN on a fixed term contact until 31 March 2026.

Main duties of the job:

  • You will be based at a maximum of two practice sites but will see patients from across all 3 of our GP practices.
  • To undertake duties governed by the PMS & GMS contract and Objectives of Direct and Local Enhanced services including:
  • Face to face and telephone consultations
  • Patient Home visits and Nursing Home visits
  • Checking and signing repeat prescriptions
  • Administration - dealing with queries, patient test results, referral letters NHS/private, task, emails and other paperwork and correspondence in a timely manner.
  • Cover all the clinical management of the patients in surgery as appropriate including initiating investigations, reviewing results, making referrals to secondary care or to other providers as appropriate.

Job responsibilities:

  • Clinical Responsibilities - In accordance with the Practice timetable, as agreed, the post-holder will be available to undertake a variety of duties including surgery consultations, telephone consultations, visiting patients at home, checking and signing repeat prescriptions and dealing with queries, paperwork, reports and correspondence in a timely fashion.
  • Triage patients presenting to the surgery by telephone, online and in person.
  • Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation.
  • Assessing the health care needs of patients with undifferentiated and undiagnosed problems.
  • Screening patients for disease risk factors and early signs of illness.
  • In consultation with patients and in line with current Practice disease management protocols, developing care plans for health and promoting healthy lifestyle strategies and education.
  • Manage long-term conditions and patients with complex problems.
  • Recording clear and contemporaneous consultation notes to agreed standards.
  • Collecting data for audit purposes and complying with QOF requirements.
  • Delivering enhanced services.
  • Check, manage & process patient test results.
  • Refer patients to secondary care and/or relevant organisations and internally within the PCN (eg to additional roles members of the PCN team) as required.
  • Compiling and issuing computer-generated acute and repeat prescriptions (avoiding hand-written prescriptions whenever possible) in line with local and national guidelines.
  • Prescribing in accordance with the Practice prescribing formulary (or generically) whenever this is clinically appropriate.
  • In general the post-holder will be expected to undertake the duties and responsibilities associated with a GP working within primary care in a surgery on a PCN basis.

  • Awareness of and compliance with all relevant Practice policies/guidelines, e.g. prescribing, confidentiality, data protection, health and safety.
  • A commitment to life-long learning and audit to ensure evidence-based best practice.
  • Contributing to evaluation/audit and clinical standard setting within the organisation.
  • Contributing to the summarising of patient records and read-coding patient data.
  • Attending training and events organised by the Practice or other agencies, where appropriate and suitable and relevant for your role.
  • Keep up to date with current evidenced based approaches to patient care and service delivery, in line with NICE and national frameworks.
  • Contribute to education and training of trainee members of staff and students.
  • Attending and contributing to practice and PCN meetings.

Person Specification:

  • Qualifications Essential:
  • MBBS or equivalent medical degree.
  • Full registration with GMC.
  • Certification of Completion of Training - JCPTGP/PMETB.
  • Inclusion on Performers List.
  • Evidence of a commitment to on-going personal & professional development.
