The GoFollow Nurse supports in-center providers in delivery of patient care by managing clinical laboratory and radiology results, answers incoming calls from patients/customers and conducts outreach to patients to relay details on patient visit and changes to result management.
Responsible for answering inquiries and questions, handles complaints, troubleshoots problems and provides information for patients/customers, and escalating cases to GoFollow APPs that require medication management changes.
Education
- Graduate from an accredited registered nursing program.
Work Experience
3+ years of experience of clinical practice experience required
Emergency Medicine/Urgent Care preferred
Previous exposure and awareness on how to deliver lab and radiology results to our patients
Required Licenses/Certifications
Active Registered Nursing State License (IN)
Additional Knowledge, Skills And Abilities Required
Proficient in relevant computer applications/electronic medical records
Strong foundation in HIPAA and compliance
Ability to type quickly, basic computer knowledge
Proficient in Microsoft Word and Excel
Detail oriented, able to rely on experience and judgment to perform a variety of tasks
Exceptional written and verbal communication skills
Able to interact and treat all persons with fairness, respect and sensitivity to cultural/social differences
Experience with electronic medical records
Required language proficiency
Knowledge of administration and clerical processes
Knowledge of call center telephony and technology

Essential Functions
Essential functions are those tasks, duties and responsibilities that comprise the means of accomplishing the jobβs purpose and objectives. Following are the essential functions of the job.
- Responsible for review and managing patient results for various labs and imaging.
- Document all activities and communications in the EMR
- Responsible for escalating cases to GoFollow APPs that require medication management changes.
- Responsible for timely management of critical abnormal lab results (within 12-24 hours)
- Responsible for relaying results and care plan to patients, explaining results and answer questions from patients
- Respond to escalated questions and concerns as needed.
- Respond to patient inquiries both telephonically and by email regarding patient visit and follow up.
- Prioritizes results and triaging messages efficiently
- Responsible for outreach for patient follow-up
- Responsible for pro rata share of weekend and on-call duties
All other duties as assigned.
