We have a new opportunity for a Patient Care Navigator to join our busy, happy Team here at Godolphin Health.

We are looking for an enthusiastic and organised person to carry out this role.

The post holder will be responsible for undertaking a range of tasks all related to patient information.

You will require excellent customer service skills as your role will involve direct face to face contact with patients.

Tasks also involve patient contact via telephone and the use of our various electronic medical information systems.

Therefore, competent experience on the use of EMIS, DOCMAN and KLINIK is essential.

The role will rarely include travelling between sites but to cover this possibility a valid driving licence is necessary.

Main duties of the job

The basic role of a GP Practice Patient Care Navigator involves a variety of tasks that are essential for the smoothoperation of the Practice.

Patient Care Navigators are the first point of contactfor patients, providing excellent customer service and administrativesupport.

They greet patients, handletelephone and email enquiries, book, amend and cancel appointments and maintainpatient records.

They also support the Clinicians with various tasks.

Patient Care Navigators play a crucial role in ensuring that patients receive the care theyneed efficiently and effectively.

About us

We are a dynamic, supportive Team with a strong reputation for high-quality patient care, excellent teamwork and a welcoming culture.

You will be joining a well‑organised Practice where the Team are valued and supported.

Godolphin Health is a two site Practice, (one at Praze an Beeble and the other at Connor Downs), conveniently situated within a 10 mins drive off the A30.

We are a training practice looking after a list of 9303 patients over two sites, dispensing to 65% of them.

The 3 partners are supported by a growing fantastic multidisciplinary Team comprising 5 salaried GPs, 1 advanced nurse practitioner, 1 paramedic practitioner, 2 practice nurses, 2 HCAs and a phlebotomist.

In addition, we have our GP trainees.

Through our Primary Care Network, we have a share of 3 mental health practitioners, a first contact physiotherapist, care home paramedic, 2 social prescribers, 2 pharmacists and 3 pharmacy technicians.

To support the above we have a very experienced and knowledgeable Team of Managers, Dispensers, Administrators and Patient care navigators.

All in all a friendly and supportive bunch!

We hold a three‑monthly protected half‑day clinical governance/educational meeting for all Team members.

  • MAIN RESPONSIBILITES
  • To liaise with team members in providing an excellent reception service.
  • To be efficient and courteous when answering enquiries from patients.
  • Receive incoming telephone calls, answer general enquiries and transfer or document calls and messages as appropriate.
  • Book appointments and telephone consultations for patients.
  • Receive and record requests for home visits.
  • Organise requests for ambulances, outpatient visits, social services and urgent test results as required by clinical staff.
  • To receive visitors and deliveries made to the surgery.
  • To be aware of other local resources and community organisations and provide information to patients.
  • To provide cover for colleagues on leave.
  • Register new patients according to the practice area.
  • Amend patients records to show and changes.
  • To be responsible for allocated tasks within the system.
  • Carry out end of day protocols ensuring reception is stocked, tidy, necessary equipment turned off and the security of the building checked.

Experience

  • ESSENTIAL SKILLS
  • Excellent IT Skills with the ability to competently navigate our medical information systems - EMIS, DOCMAN and KLINIK.
  • Ability to drive.

You will be on a rota covering 2 surgery sites, that are 4.5 miles away from each other.

However, you will rarely be asked to travel from one site to another within a course of a day

  • Experience of working in a GP Practice.
  • Disclosure and Barring Service Check
