The predicted salary is between 36000 - 60000 £ per year.

To work as a member of the Home First/Reablement Team with people in their own homes, for a time limited period after a hospital admission (Home First) assisting them to re-settle, providing assessments and interventions if needed to ensure no or optimum levels of support are provided ongoing.

To work with people in their own homes referred to Reablement, which is a time limited and goal based service assisting people to retain or to gain confidence, skills and abilities to live well at home.

Working with Therapists and Reablement Co-ordinator to undertake assessments and provide review to ensure people are receiving the right level of input and/or are working on their outcome based goals.

You will need to hold a valid driving licence and have use of a vehicle to enable you to drive around the locality so you can work with people in their homes. Team working and communication skills are essential. You will be confident with handling IT systems to update written notes and electronic records.

Working with Reablement Co-ordinator to support and maintain quality standards of work delivered by Reablement Workers.
