About The Role
As a Commissioning Broker, you will be at the heart of connecting people with the care they need. Acting as the link between families, professionals, and providers, you will help secure high-quality care packages and placements that truly make a difference.
In This Role, You Will:
- Support safe and timely hospital discharges, working closely with partners to ensure everything is in place.
- Manage a diverse and sometimes complex caseload.
- Maintain accurate and up-to-date records across systems.
- Handle sensitive information with professionalism and confidentiality.
- Monitor provider performance to maintain high standards of care.
- Work collaboratively with managers to improve processes and outcomes.
