Advanced Nurse Practitioner in Glossop

We are seeking an enthusiastic and experienced Advanced Nurse Practitioner (ANP) to join Glossop Primary Care Network (PCN) for 30 hours per week, working across our four GP practices. This is an exciting opportunity to play a key role in delivering high‑quality, patient‑centred care and supporting accessible, efficient primary care services for our local community.

Working as part of a supportive multidisciplinary team, you will assess, diagnose, treat and manage patients with a wide range of acute and long‑term conditions. By working across all four practices, you will help ensure consistency of care, share clinical expertise and contribute to the PCN’s commitment to integrated, proactive and equitable healthcare. You will undertake clinics within our practices and, when requested by practice, complete home visits within the community. Flexibility in this role is key.

Main duties of the job:

• The Advanced Nurse Practitioner (ANP) provides autonomous, evidence‑based clinical care for patients presenting with acute illness, minor injury and long‑term conditions.
• Key responsibilities include undertaking comprehensive assessments, diagnosing and managing health conditions, requesting and interpreting diagnostic investigations, prescribing medication where qualified, and developing personalised treatment plans within clinics and home visits.
• The ANP works collaboratively within the multidisciplinary team, provides health promotion and preventative care, supports the management of complex patients, contributes to service development and quality improvement initiatives, and maintains accurate clinical records in accordance with professional and organisational standards.

Job responsibilities:

• Provide assessment, diagnosis and treatment at first point of contact by attending to patients according to their needs either in the practice sites or patient homes.
• Act as an autonomous clinician, with the support and guidance from the GPs.
• Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long‑term condition (as appropriate).
• Diagnose and manage both acute and chronic conditions, integrating drug and non‑drug based treatment methods into a management plan.
• Recognise, assess and refer patients presenting with mental health needs.
• Evaluate clinical information from examination and history taking and initiate appropriate treatment and/or referral, including admission to hospital if required.
• Signpost appropriately to other services in the identified healthcare pathway and refer patients to appropriate care settings, within locally agreed healthcare pathways.
• Maintain accurate and systematic documentation of patient interactions and clinical care provided, including adverse events.
• Advise and support members of the public on healthy lifestyles, health promotion and disease prevention.
• Provide clinical advice to other members of the team where appropriate.
• Support the GPs in other duties as required, for example assisting with ECGs or other nursing duties where a nurse is not available.

• Evidence of excellent communication skills including diplomatic and negotiation skills

• Significant post‑registration experience in primary care, community, or acute settings
• Proven ability to work flexibly to suit the needs of the practices
• Proven ability to work autonomously

• Understanding of health and social care processes
• Understanding of social determinants of health and how these can be addressed with patients
• High levels of health literacy
• Awareness of local services and resources for individuals, carers and families
• Knowledge of a range of technology and digital tools that can be used to support health and wellbeing
• Sound understanding of disease prevention (primary, secondary and tertiary) and how patients can build prevention into their lifestyle
• Excellent knowledge of Microsoft Office and EMIS

• Registered Nurse with valid NMC registration
• MSc in Advanced Clinical Practice
