Occupational Therapy Job Description
- Speciality -Band 6 Community/RR OT
- Location -Ilford
- Start date -ASAP
- Contract Length - Ongoing
- Pay Rate - £26-28 per hour
- 5 x Band 6 Community/RR OTs
Responsibilities / Activities
- Provide urgent OT assessment and intervention to prevent unnecessary hospital admissions.
- Deliver rapid response support to stabilise individuals during periods of acute decline.
- Assess functional ability, falls risk and environmental factors within patients' homes.
- Implement immediate strategies and equipment solutions to maintain independence safely.
- Work closely with MDT colleagues to provide crisis intervention and urgent care planning.
- Support safe discharge pathways and reduce avoidable delays in transfer of care.
- Liaise with health, social care and voluntary services to coordinate community support.
- Develop person-centred plans that maximise function and reduce admission risk.
- Monitor outcomes and adjust interventions to support recovery in the community.
- Maintain accurate clinical records and contribute to service performance targets.
Skills / Experience
- Car drivers required given the community role across 3 London boroughs
- Experience in NHS and post qualification as an OT, preferrably within the community and rapid response setting.
Requirements
- 2 year(s) post graduate experience
- Valid Passport/Visa
- Fully compliant and cleared to work
- DBS or Police Check
- HCPC Registered
