GLL is seeking a Healthwise Facilitator to work across sport and leisure facilities in Preston. This role is ideal for individuals with the skills and ambition to develop their career in health and fitness, offering more than just a job but a professional pathway.
The Healthwise Facilitator will be responsible for prescribing suitable physical activities and supporting behavior change for Medically Referred clients. Responsibilities include setting up and conducting group and individual training sessions, managing the GP Referral programme, including referral processing, record keeping, and data management.
The role reports to the Healthwise Coordinator / Health Manager.
What you’ll do:
- Ensure the smooth operation of the Physical Activity Referral Scheme (PARS), optimizing resource use.
- Conduct health and lifestyle assessments for referral patients and develop safe, appropriate physical activity programs.
- Act as the first point of contact for scheme participants.
- Provide evidence-based, culturally sensitive health and lifestyle information.
- Collaborate with team members and support services within the organization.

- Level 3 Fitness Instructor or Personal Training qualification (Essential)
