This is an excellent opportunity to join our team of dynamic and forward‑thinking administrators.
Working with the LTC (Long Term Condition) team, you will co‑ordinate the call and recall of specific cohorts of patients.
The successful candidate will be well organised, have good interpersonal skills and have the ability to work under pressure.
Previous experience of working in general practice and knowledge of patients would be highly desirable.
Cross site working is essential.

Main duties of the job
We are looking for someone who can work as part of our team but can also work on their own initiative in a concise and methodical way.
This role will primarily involve contacting specific cohorts of patients who need regular reviews and therefore the successful candidate must be an effective communicator with a good telephone manner.

Glenpark & Teams Medical Partnership has been serving the population of Dunston, Teams and the surrounding area for over 100 years.
The Practices are situated with easy access to the A1 and the Metro Centre.
The Practice boundary covers Dunston, Whickham, Swalwell, Lobley Hill, Teams and some of Bensham.
- Job responsibilities
- The below list is not exhaustive but contains some of the main duties of the role:
- Work closely with the clinical and nursing team to identify specific cohorts of patients requiring reviews for long term conditions.
- Ensure recall lists are kept up to date and accurate information is held.
- Navigate the practice appointments system ensuring appropriate slots are available and assigned following LTC protocols.
- Record and pass on electronic messages to district nurses, health visitors and other staff where appropriate.
- Arrange follow‑up appointments with doctors, nurses and other health professionals as required.
- Answer all telephone calls in a courteous manner and deal with them appropriately.
Person Specification
- Knowledge and Skills
- Excellent communication skills, both written and oral.
- An understanding, acceptance and adherence to the need for strict confidentiality.
- Ability to work under pressure and prioritise workload.
- Good time management skills and able to work to deadlines. Availability to cover sickness & holidays.
- Good interpersonal skills.
Qualifications
- GCSE or equivalent in English and Mathematics.
- Proficient use of Microsoft Word, Excel, Outlook and other MS Office applications.
Experience
- Proven experience of working with patients/public.
- Experience of handling confidential information.
- Practical experience of working within a team.
- Experience of working in a General Practice or NHS environment. Knowledge of EMIS Web clinical systems.
