Responsibilities

  • Support and enable the People We Support to live their best life, delivering person‑centred care unique to each individual, aligned with our person‑led support delivery model.
  • Use your knowledge, skills and adherence to legislation, best practices and our policies and procedures to deliver the highest quality care.
  • Engage with individuals to enable them to pursue their interests, maintain their skills and receive personalised, person‑centred care.
  • Contribute to the development and maintenance of relationships and connections within their local community.
  • Complete and maintain Support Plans and other necessary information to support our people and ensure effective delivery of support.
  • Notify the Team Leader/Senior Support Worker of any changes in the needs of individuals we support.
  • Safeguard the welfare of those in our care and actively promote and maintain their safety.
  • Help the people we support to maintain and develop relationships with family, friends and other important people in their life.

Requirements

  • Driver preferred; all checks fully funded by Glassmoon Services Ltd.
  • Right to work in the UK.
  • Satisfactory Criminal Record Checks.
  • References.
  • ID.
