About the role
As a Support Worker, you’ll play a meaningful role in helping young individuals with complex needs to build and maintain meaningful connections within their community, achieve desired outcomes, and develop the skills they need to live independently. You will work alongside a dedicated team that values inclusivity, respect, and personal growth.
Responsibilities
- You will use your knowledge, skills, and adherence to legislation, best practices, and our policies and procedures to deliver the highest quality care.
- You will enable each person we support to engage with their interests, maintain their skills, and receive personalised, person‑centred care.
- You will actively engage with those we support to contribute to the development and maintenance of their relationships and connections within their local community.
- You will complete and maintain Support Plans and other necessary information, ensuring the delivery of effective support.
- You will notify the Team Leader/Senior Support Worker of any changes observed in the needs of individuals, contributing to their well‑being.
- You will safeguard the welfare of those in our care by actively promoting and maintaining their safety.
- You will help people maintain and develop relationships with family, friends, and others important in their lives.
