Support Worker – Glassmoon Services
We are looking for a caring and dedicated Support Worker to provide 24‑hour support to young people with autism and/or learning disabilities. The role involves enabling them to live as independently as possible, build community connections, and pursue personal goals such as education, employment, and leisure activities.
Responsibilities
- Deliver person‑centred care that aligns with the individual’s support plan and NHS standards.
- Engage with clients to help them pursue their interests, maintain skills, and access tailored support.
- Support the development of relationships and community connections for each person.
- Maintain accurate support plans and other records to ensure effective service delivery.
- Communicate any changes in a client’s needs to the team leader or senior support worker.
- Safeguard the well‑being and safety of all individuals in care.
