Responsibilities

  • Deliver person‑centred care that supports each individual’s unique needs and interests.
  • Develop and maintain Support Plans, ensuring accurate and up‑to‑date information.
  • Facilitate meaningful community engagement, relationships, and skill development.
  • Observe and report changes in an individual’s needs to the Team Leader/Senior Support Worker.
  • Safeguard the welfare and safety of all supported individuals.
  • Routinely complete required paperwork, such as incident and progress reports.

Qualifications & Requirements

  • Right to work in the UK.
  • Satisfactory criminal records checks, ID, and address verification.
  • Drivers licence preferred (funded by Glassmoon Services Ltd).
  • Reference checks.
