We are recruiting for Support Workers for a new Supported Living Service based in Manadon, Plymouth. Initially, this role will involve supporting the team with transition work, whilst the property is being remodeled.
Are you experienced in working with individuals who have a Learning Disability and Mental Health needs? We are searching for a team of highly skilled and passionate individuals to assist someone in embarking on a new chapter in their life. This individual will be moving to Plymouth and is eager to actively engage in the community and explore fresh opportunities. This role is both demanding and incredibly fulfilling, as you will play a crucial part in helping this person strive for their best life.
Your responsibilities will encompass supporting them in participating in all the activities they find enjoyable, while fostering their independence to the fullest extent. Possessing a valid driver's license will be advantageous.

As a Support Worker, you'll play a meaningful role in:
- Supporting and enabling the People We Support to live their best life, whilst delivering person-centered care, unique to each individual, aligned with our person-led support delivery model.
- Using your knowledge, skills, and adherence to legislation, best practices, and our policies and procedures to deliver the highest quality care.
- Enabling individuals to engage with their interests, maintain their skills, and receive personalised, person-centered care.
- Actively engaging with the people we support to contribute to the development and maintenance of their relationships and connections within their local community.
- Completing and maintaining Support Plans and other necessary information that supports our people and ensures the delivery of effective support.
- Notifying the Team Leader/Senior Support Worker of any changes you observe in the needs of the individuals we support, contributing to their continued well-being.
- Safeguarding the welfare of those in our care and actively promoting and maintaining their safety.
- Helping the people we support to maintain and develop relationships with family, friends and other important people in their life.
