We are recruiting Support Workers based in Plymouth to assist individuals with a Learning Disability and complex needs.
Responsibilities
- Support individuals to live their best life, delivering person‑centred care aligned with our model.
- Use knowledge, skills, and adherence to legislation and best practices to provide high‑quality care.
- Enable individuals to engage in interests, maintain skills and receive personalised care.
- Engage with people to support relationships and connections within their local community.
- Maintain support plans and documentation accurately to ensure effective support.
- Notify the Team Leader/Senior Support Worker of any changes in needs.
- Promote and maintain safety (safeguarding).
- Help maintain and develop relationships with family, friends and other key people.
