What We're Looking For:

  • Proven experience in the health and social care sector, ideally within supported living services.
  • A Level 3 (or higher) qualification in Health & Social Care (or equivalent).
  • Strong understanding of CQC regulations, safeguarding, and person-centred care.
  • Confident leader with excellent organisational and communication skills.
  • Flexible and able to work occasional evenings and weekends.
  • Proficiency in IT, including Electronic Care Management Systems.
  • Experience or knowledge in Positive Behaviour Support (PBS), Trauma-Informed Care, Mental Health, Learning Disabilities, and behaviours that challenge would be a significant advantage.

Key Responsibilities:

  • Deliver person-centred care and support, ensuring the well-being of service users and helping them to achieve their individual goals.
  • Coordinate care plans, and collaborate with service users, families, and external agencies to provide tailored support.
  • Monitor the effectiveness of care plans and make adjustments as necessary.
  • Ensure compliance with CQC standards and safeguarding requirements.
  • Provide leadership and support to care staff, fostering a collaborative team environment.
  • Maintain accurate records using Electronic Care Management Systems.
  • Work with service users who may display challenging behaviours, applying PBS and Trauma-Informed Care approaches.
  • Flexible scheduling, including evening and weekend shifts.

What You'll Bring:

  • Proven experience as a Care Coordinator, ideally within supported living or similar settings.
  • A Level 3 (or higher) qualification in Health & Social Care, or an equivalent qualification.
  • Strong understanding of CQC regulations and person-centred care principles.
  • Excellent leadership, communication, and organisational skills.
  • Proficiency in IT and Electronic Care Management Systems.
  • Knowledge of Positive Behaviour Support (PBS), Mental Health, Learning Disabilities, and behaviours that challenge would be highly desirable.
  • A full, clean driving license and access to your own vehicle.
