What We're Looking For:
- Proven experience in the health and social care sector, ideally within supported living services.
- A Level 3 (or higher) qualification in Health & Social Care (or equivalent).
- Strong understanding of CQC regulations, safeguarding, and person-centred care.
- Confident leader with excellent organisational and communication skills.
- Flexible and able to work occasional evenings and weekends.
- Proficiency in IT, including Electronic Care Management Systems.
- Experience or knowledge in Positive Behaviour Support (PBS), Trauma-Informed Care, Mental Health, Learning Disabilities, and behaviours that challenge would be a significant advantage.
Key Responsibilities:
- Deliver person-centred care and support, ensuring the well-being of service users and helping them to achieve their individual goals.
- Coordinate care plans, and collaborate with service users, families, and external agencies to provide tailored support.
- Monitor the effectiveness of care plans and make adjustments as necessary.
- Ensure compliance with CQC standards and safeguarding requirements.
- Provide leadership and support to care staff, fostering a collaborative team environment.
- Maintain accurate records using Electronic Care Management Systems.
- Work with service users who may display challenging behaviours, applying PBS and Trauma-Informed Care approaches.
- Flexible scheduling, including evening and weekend shifts.
What You'll Bring:
- Proven experience as a Care Coordinator, ideally within supported living or similar settings.
- A Level 3 (or higher) qualification in Health & Social Care, or an equivalent qualification.
- Strong understanding of CQC regulations and person-centred care principles.
- Excellent leadership, communication, and organisational skills.
- Proficiency in IT and Electronic Care Management Systems.
- Knowledge of Positive Behaviour Support (PBS), Mental Health, Learning Disabilities, and behaviours that challenge would be highly desirable.
- A full, clean driving license and access to your own vehicle.
