About the Role: As a Care Coordinator, you will be a key player in ensuring seamless care delivery. This role involves coordinating care packages, managing rotas, liaising with service users and families, and ensuring compliance with all relevant CQC regulations. You\’ll be joining a passionate team committed to delivering outstanding care and support.
