The post holder will be responsible for the delivery of basic practice nursing services within the practice.They will work with support from senior practice nurses and the clinical nurse manager to deliver highquality care. They will work within all aspects of practice nursing after completion of training andcompetencies. Supporting patients to be healthy and participating in the monitoring of patients with longterm conditions and health screening.The post holder will work under local and practice policies to ensure patients health needs are met andproviding support to junior nurses, healthcare assistants, phlebotomists and student nurses as required.
Hours required if possible –
Tuesday 2pm -6pm
Thursday 1-6pm
Friday 8am – 6pm
Main duties of the job
1. Assess, plan, develop, implement and evaluate programmes to promote health and well- being ofpatients attending the practice
2.Participate and undertake opportunistic and routine cardiovascular risk assessments, providinghealth advice and making appropriate referrals into exercise, alcohol, smoking, weightmanagement and mental health services where applicable.
3.Work with patients in order to support compliance with and adherence to prescribed treatments.
4.Support patients to adopt health promotion strategies that promote healthy lifestyles, and applyprinciples of self care.
5.Support and manage health needs of women presenting for family planning, cervical screeningand sexual health consultations (cervical screening after completion of course)
6. Assess, identify and refer patients presenting with mental health needs. Participate in annualmental health reviews
7.Undertake ECGs as per practice policies from the Nurse or GP and venepuncture
8.Administer medications or injections as per prescription, patient group directions or patientspecific directions.
9.Assess, plan, implement and evaluate treatment for patients presenting with acute and chronicwounds. Including removal of clips and sutures.
10.Undertake the collection, storage and despatch of pathological specimens to a high standard(blood samples, urines, stool, sputum, swabs, smears etc) ensuring universal precautions arefollowed and manage the results of these samples, referring to GP as necessary.

- Previous experience of primary care
- 5 GCSE A-C
- Qualified in phlebotomy
- Hours required to work if possible

- Worked as a practice nurse in a primary care setting
