Position: Registered Nurse – RN – Hospice – PRN – Flexible Schedule – This position will serve as backup for On Call duties – Must be available for after-hours shifts from 5 p.m. to 8 a.m. Responsibilities of a Hospice RN/ RN Case Manager: Report directly to the Administrator, Executive Director, or Patient Care Manager and maintain primary control and professional management of each patient. Act as the primary liaison between the physician, patient/family, and hospice team. Provide direct patient care services to hospice patients and supportive care to their families, managing a reasonable caseload with compassion and trust. Empower caregivers through education on patient care. Collaborate with an interdisciplinary team including LPNs, CNAs, and support staff to provide comfort, peace, and dignity. Evaluate patient and family needs and assist in developing and implementing care plans. Maintain all clinical records accurately. Participate in quality assessment and performance improvement programs. About You Hospice Registered Nurse Requirements: Compassionate, kind, and empathetic towards patients and families. Comfortable providing care in the patient\’s home environment. Flexible and able to support others during crises. RN license in the state of employment or eligibility to obtain one. At least 1 year of nursing experience in hospice, home health, ICU, geriatric, or oncology is preferred. Additional experience in hospital settings such as Med Surg, ER, ICU, or critical care is advantageous. Valid driver’s license and automobile insurance. Current CPR certification.
