Responsibilities

- Facilitate group sessions and workshops that encourage peer support and share learning around the four pillars of wellbeing: sleep hygiene, eating habits, exercise, and wellbeing strategies.
- Accurately record and report outcomes using appropriate systems, contributing to service evaluation and continuous improvement.
- Manage a caseload of patients, maintaining regular contact and providing ongoing support to track progress, adjust plans, and celebrate successes.
- Collaborate with multidisciplinary teams, including GPs, social prescribers, and mental health professionals, to ensure a joined‑up approach to patient care.
