A rewarding opportunity for a Health and Wellbeing Coach to work in community settings across the South Birmingham Locality providing one to one and group support to patients with low level mental health support needs. You will work closely with other support staff and organisations including other Gateway services such as Social Prescribers, counsellors, and other mental health professionals. You will provide low intensity interventions, supporting patients to manage their own recovery.
Main Duties of the Job:
- Provide one to one and group based support to patients referred by mental health and community services as well as self referrals.
- Support will focus on motivational interviewing, goal setting, and action planning with support spanning a shorter time frame of around 6 appointments.
- Maintain a caseload and accurate record keeping.
- Work as part of a team of Health and Wellbeing Coaches and other mental health support staff across South Birmingham.

Job Responsibilities:
- Conduct patient‐centred interviews to identify areas where patients wish to see change and agree action plan and steps in conjunction with patients.
- Make accurate assessments of the risk patients pose to themselves and others.
- Make decisions on the suitability of new referrals, referring clients to alternative services or 'stepping up' their treatment to high‐intensity psychological therapy if necessary.
- Devise a shared treatment plan with your patient delivered via a range of methods such as face‐to‐face, online, or over the telephone.
- Provide low‐intensity interventions using current methods, models, and tools to influence positive behaviour change.
- Provide both group and one to one support to patients using a range of mediums including face to face and telephone support.
- Attend multidisciplinary meetings about referrals or clients in treatment.
- Develop strong professional relationships with partner colleagues, Gateway staff teams, and external support services.
- Liaise with external agencies including housing, police, local authority, employers, and employment support workers where necessary.
- Provide and receive information related to mental health to individuals or groups of clients, relatives, carers, members of the public, and professionals.
- Plan and prioritise own workload to ensure appropriate levels of intervention are provided to each client.
- Complete and maintain client records and update databases to provide key outcome and evidence monitoring of service.
- Signpost and refer to other agencies and services to promote sustained behaviour change and health outcomes.
- Design and complete relevant paperwork to provide support to clients in the form of assessments, updates, and action plans.

Person Specification:
- Able to develop good therapeutic relationships with clients.
- Demonstrate a good understanding of the evidence base and development of self‐management.
- Excellent teamworking skills and an aptitude for fostering good working relationships at all levels.
- An interest in using clinical supervision and personal development positively and effectively.
- Able to work well under pressure and effectively manage a caseload of multiple patients.
- Knowledge of safeguarding (Adults and children) and risk assessments.
- Excellent facilitation skills including conflict resolution in group settings.
- Active listening skills with an ability to build rapport with people and patients.
- Able to communicate clearly and persuasively, both verbally and in writing.
- Basic IT skills, including word processing and database packages.
- Good time management and organisational skills relating to the overall number of client contacts offered and clinical sessions carried out per week.
- IT skills to include Microsoft Office.
- Understanding of long‐term conditions and their impact on wellbeing and motivation.
- Knowledge of health and wellbeing services and support in Solihull.
Experience:
- Experience of supporting patients with low level mental health issues and conditions.
- Experience of using coaching approaches and techniques or other supportive strategies (e.g., Motivational Interviewing).
- Experience of providing one‐to‐one and group support based on patient need.
- Managing a caseload of patients, prioritising in accordance with need and support required.
- Working as part of a multi‐disciplinary team, working alongside internal and external agencies to provide comprehensive support.
- Development of shared treatment plans in coproduction with patients.
- Planning and prioritising own workload.
- Accurate record keeping in relation to patient records, referrals, and outcomes.
- Experience of working within or alongside Social Prescribing Services.
- Working as part of a multidisciplinary Primary Care team.
Qualifications:
- Coaching/Counselling qualification gained through a PCI accredited organisation (or be willing to work towards) (H&WB Coach 4 day training).
- Mental Health based qualification – e.g. NVQ Level 2 or above.
- Registered member of an appropriate governing body (BACP, BABCP).
