We are looking for a skilled and motivated Registered Mental Health Nurse to join our Community Mental Health Wellness Team at NELFT. This role offers the opportunity to work autonomously within a supportive multidisciplinary team, delivering high‑quality, recovery‑focused care to adults with complex mental health needs. You will act as a named key worker, playing a central role in assessment, care planning and coordination of care to improve outcomes and support independence.
Key Responsibilities
- Act as a named key worker for a defined caseload, coordinating care and supporting recovery‑focused outcomes
- Undertake comprehensive mental health assessments and develop, implement, and review care plans
- Deliver evidence‑based interventions for individuals with complex and co‑existing needs
- Safely administer, monitor and review medication in line with NMC and Trust standards
- Assess and manage clinical risk, escalating concerns appropriately and contributing to risk management processes
- Work collaboratively with GPs, primary care and partner agencies to support safe discharge and continuity of care
- Maintain accurate, timely clinical records to support care delivery and audit requirements
Qualifications
- Registered Mental Health Nurse with current Nursing and Midwifery Council registration
- Experience working with individuals with complex mental health needs in a community setting
- Strong clinical decision‑making, risk assessment and care coordination skills
- Commitment to recovery‑focused, person‑centred care
- Ability to work both independently and as part of a team
