An exciting opportunity is available to apply for the position of Nursing Team Lead within the Community Rapid Response Nursing Team. This opportunity is only available to current Band 7 Senior Practitioners who currently work in the nursing team. The post holder will be required to provide strong leadership to a large nursing team who are responsible for providing urgent community response nursing care to the residents of Gateshead. The team also provides unplanned nursing care, planned nursing care and out of hour palliative care within the remit of current Community pathways.

Applicable candidates should have extensive experience working within the Rapid Response Team and thus will have a strong clinical background in this area of practice. The applicant should also be able to demonstrate strong leadership and management skills to lead the team throughout the current period of change and development. The successful candidate will be required to work as a team lead and a senior practitioner providing both clinical leadership and managerial support to the rest of the team. There will be a requirement to work over 7 days per week, 365 days per year, including weekend and bank holiday working.

Main duties of the job

This is an exciting opportunity for staff to lead and develop a service they are passionate about. This post will be based in Bensham Hospital working alongside the Rapid Response Multidisciplinary Team. The successful candidate will be responsible for the leadership and management of the Rapid Response Nursing Team. They will also be required to work as an autonomous senior practitioner to meet the clinical requirements of the service. The post holder will ensure that the care being delivered is person centred, safe and effective.

  • Be professionally registered with an appropriate governing body
  • Hold a degree in a health related subject
  • Be educated to masters level or willingness to work towards
  • Possess a teaching qualification
  • Have physical assessment skills and an ability to work autonomously as a senior practitioner
  • Significant experience in leadership and management

Job responsibilities

To be accountable for the overall leadership, management and development of the 24/7 Rapid Response Nursing Team within Gateshead Community Services. To work effectively within a multi-disciplinary/agency team managing patients identified as requiring an urgent community response to prevent their health and wellbeing deterioration. In addition, there will be planned activity to provide hands on care and holistic assessment to prevent avoidable admission to hospital, and/or long-term care and to maximise patient independence to support safe effective service delivery. This post is designed as a senior clinical autonomous practitioner and leader who will ensure the fundamentals of care are provided at the point of contact and will ensure delivery of a range of appropriate interventions to be able to meet the needs of the patients health and wellbeing, within the community setting. Patients accessing the service will have complex needs and potentially numerous co-morbidities including frailty and this post is the senior clinical decision maker and leader in a 24/7 rapid response nursing team within Gateshead. This post will be responsible and accountable for the safe delivery of a Rapid response nursing service 24/7 for the patients of Gateshead and will support the Clinical Operations Manager in the safe and effective delivery of their role in unscheduled, palliative and intermediate care.
