There is an exciting opportunity available for a current Band 6 nurse working within Acute Frailty/Frailty Virtual Ward to apply for a Band 7 Frailty Nurse Practitioner Development Post. The successful candidate will commence the post as a Band 6 Frailty Nurse and work towards a set of defined objectives, with training and support, with the aim of developing into a Band 7 Frailty Nurse Practitioner within a 12 month period.
Main duties of the job
- Work as a Frailty Nurse Practitioner within the Acute Frailty team and Frailty Virtual Ward team on a rotational basis.
- Use physical assessment skills and non-medical prescribing to assess, treat and manage patients under the care of Acute Frailty/Frailty Virtual Ward.
- Work closely with members of the MDT and wider acute/community services, ensuring the development of good relationships.
- Contribute to the delivery and development of the frailty services using effective evidence-based practice in line with agreed national standards for clinical practice.
Job responsibilities
- Participate in the planning, development and evaluation of clinical services using advanced clinical skills and knowledge.
- Provide specialist advice on the assessment, care planning and interventions of patients' needs.
- Enhance knowledge and skills through regular audits, research, and development activities in community, acute settings, and virtual wards.
- Collaborate across roles and organisational boundaries to support the Frailty service, referring to social and health services for comprehensive patient management and to optimise outcomes.
- Work autonomously and collaboratively within a multidisciplinary team, managing at-risk patients with complex needs and co-morbidities, including frailty.
- Engage in cross-agency liaison, training, research, and development, serving as a clinical resource for the local Health Community.
- Maintain clinical responsibility for a defined complex caseload, ensuring accountability for health outcomes.
- Develop, deliver, and manage complex case management within the community/acute setting and virtual wards.
- Ensure safe systems and processes for multidisciplinary team working.
- Adhere to all professional standards and codes of conduct.
- Facilitate the development of clinical expertise and advanced practice within the team.
- Provide senior leadership and clinical expertise in caseload management for multidisciplinary staff.
- Oversee and monitor the standard of care, ensuring high-quality assessment, planning, implementation, and evaluation.
- Act as a caseload holder, overseeing clinical care planning and delivery, delegating as appropriate.
- Lead clinically and professionally, ensuring compliance with statutory and mandatory requirements.
- Continuously develop and share clinical expertise, knowledge, and skills.
- Champion leadership competencies within the team.
- Ensure continuity and coordination of care across the health and social care community.
- Develop protocols for advanced practice and extended clinical roles.
- Review patient/client dependency, staff skill mix, caseload, and shift patterns in line with needs and staff development.
- Facilitate self-awareness and reflection within the team through clinical supervision.
Person Specification
- Currently working within Acute Frailty/Frailty Virtual Ward.
- Physical Assessment Skills Qualification.
- Willingness to work towards defined objectives within 12 months.
