Responsibilities:
- The Social Worker will be responsible for implementing effective treatment and discharge planning based on established Standards of Practice set forth by the NY Office of Professions and the National Association of Social Workers, and ensuring all patient care policies and procedures are adhered to in accordance with Garnet Health Medical Center - Catskills policies and New York State Licensure requirements.
- The Social Worker will exhibit critical thinking, effectively organise/prioritise accordingly, and implement evidence-based care.
- Social Work Clinician/Case Manager β primary role is to provide psychiatric/detox emergency screening in the Garnet Health Medical Center - Catskills Emergency Department. Additionally, will provide case management and discharge planning for non-psychiatric patients.
- The Clinician is also responsible for obtaining insurance approval for inpatient behavioral health treatment, ensuring that correct legal papers are completed and preparing other documentation as necessary.
- Maintaining administrative records related to emergency evaluations and case management. Close collaboration with ED physician and other staff members, psychiatrist on-call, and Behavioral Health inpatient units.
- Frequent contacts with law enforcement personnel, EMS, Medical Center security, ED RN staff, and family members.
- SW Clinician β is also responsible to be part of the multidisciplinary team in addressing the needs of a behavioral health patient in medical floor by assisting in their treatment and ensuring that patient has appropriate discharge plan.
- The SW Clinician will also be the primary Case Manager for the detox patient on the medical floor assuring discharge planning, utilisation review, and insurance approval.

Qualifications:
- Minimum Education: Masterβs degree in Social Work or Mental Health Counseling.
- Minimum Experience: Demonstrated assessment skills. Ability to utilise problem-solving theories and techniques, combined with flexibility and creativity, in working with a diverse population.
- Must possess time management skills, highly developed verbal and written skills, problem-solving capacity, critical thinking, and good organisational skills. Requires knowledge of community resources, third party payers, and regulatory agencies. Competency on computer is preferred.
- Required Certification/Registration: Currently licensed and registered in NY State to practice as a social worker. BLS certification required upon hire. Must attend hospital crisis program within 3 months of hire or transfer and annually thereafter. Maintains all required certifications for the areas assigned.
Physical Requirements:
- Position will primarily be medium to light work requiring lifting and/or carrying objects weighing up to 50 pounds. Bending, twisting, stooping, kneeling, and reaching are required. Position requires 20% sitting and 80% standing or walking.
- Expressing or exchanging ideas by spoken and written word is required.
Working Conditions:
- Environmental Demands and Exposure to Hazards: Works in a clean, well-lighted, heated or ventilated facility. No routine exposure to hazards. Position may have occasional exposure to blood, body substances, infectious materials, chemical hazards, vibration, radiation, slippery surfaces, or noise that may result from performance of duties.
- Physical Demands: Demonstrates physical and functional ability to perform full anatomical range of motion to accomplish tasks. Evidence of visual and aural acuity and finger and hand dexterity to operate computer and office equipment. Can withstand long periods of sitting, standing and/or constant walking. Ability to lift 50 lbs.
- Mental Demands: Ability to foster collaborative relationships, to work well under pressure, to organise and synthesise new information, and prioritise tasks. Possesses critical thinking, analytical skills and flexibility. Ability to multi-task. Required detailed attention to work in an environment where interruptions cannot be controlled. Demonstrates sensitivity to customer needs and expectations. May be subject to irregular hours including evenings or potentially weekends to participate in operational and community events as necessary.
