About the Role

Support is provided through a flexible mix of face-to-face sessions, as well as remote contact via telephone and video link, allowing for responsive and accessible care tailored to each individual's needs.
Key Responsibilities
- Carry out person-centred assessments and develop individual support plans
- Provide one-to-one emotional and therapeutic support
- Deliver interventions in-person and remotely (telephone and video)
- Monitor outcomes, manage risk, and follow safeguarding procedures
- Liaise with families, carers, and external professionals to coordinate care
- Maintain accurate and confidential case records
- Contribute to ongoing service improvement
About You
- Degree or equivalent qualification in Social Work
- Registered with the appropriate UK regulatory body (e.g. Social Work England)
- Strong interpersonal and communication skills
- Counselling skills or therapeutic experience (desirable)
- Confident working independently and remotely
