Partners in Health (part of GDOC Ltd) is looking to recruit a highly motivated Advanced Nurse Practitioner to join our expanding primary care team. As a valued member of our multidisciplinary workforce, you will deliver high-quality, patient-centred care across our primary care services. Main duties of the job:
- The ANP/AP will work autonomously within their scope of practice to assess, diagnose, treat, and manage patients presenting to the practice.
- They will provide expert clinical care, work alongside the multidisciplinary team, and contribute to service development to meet the needs of the practice population.

Job responsibilities:
- Clinical: Undertake comprehensive assessment, diagnosis, and treatment of patients with undifferentiated and undiagnosed problems, both acute and chronic.
- Prescribe and review medication in accordance with national and local prescribing guidelines.
- Order and interpret investigations to inform clinical decision-making.
- Manage patients across the lifespan, including paediatric, adult, and elderly populations.
- Identify and manage long-term conditions, health promotion, and disease prevention strategies.
- Recognise and respond to medical emergencies and deteriorating patients.
- Work effectively across the 4 pillars of advanced practice: clinical, leadership, research, and education.
- Professional: Maintain professional registration with the appropriate regulatory body (NMC/HCPC/GPhC).
- Hold (or be eligible for) an Advanced digital badge issued by the centre for Advancing Practice.
- Work within national ACP/AP frameworks, scope of practice, and local protocols.
- Engage in clinical supervision and maintain an up-to-date professional portfolio.
- Participate in continuing professional development and annual appraisal.
- Leadership & Service Development: Contribute to quality improvement, audit, and clinical governance activities.
- Support the development of pathways, protocols, and practice policies.
- Mentor and support junior clinicians, nurses, and trainees within the practice.
- Communication: Communicate effectively with patients, carers, and other healthcare professionals.
- Document consultations clearly and accurately in the patient record.
- Liaise with community services, secondary care, and social care teams to ensure continuity of care.
Working conditions:

- High levels of accuracy and attention to detail essential at all times.
- Exposure to distressing situations and written material.
- Contact with body fluids, i.e. wound exudates; urine etc. while in clinical practice.
- Uniform/scrubs to be worn as agreed with line manager.
Person Specification:
- Experience: Essential: Experience of General Practice; Proven experience in autonomous clinical decision-making. Desirable: Minimum 2 years post-registration experience; Experience managing long-term conditions; Experience of mentoring and supervising other clinicians.
- Qualifications: Essential: Registered with relevant professional body (NMC, HCPC, GPhC); MSc Advanced Clinical Practice or NMC-recognised equivalent; Hold (or be eligible for) an Advanced digital badge issued by the centre for Advancing Practice. Desirable: Independent prescriber or working towards it.
- Other: Essential: Disclosure Barring Service (DBS) enhanced check satisfactory; Evidence of continuing professional development; An ability to drive, current UK driving licence, and daily access to a vehicle.
- Skills: Essential: Excellent communication and consultation skills; Understanding of ACP 4 pillars: clinical practice, leadership, education, research; Sound understanding of clinical governance and safeguarding; Advanced assessment, diagnostic reasoning, and treatment planning skills; Willing to become competent in use of SystmOne if not already; Ability to follow policy and procedure; Able to identify and resolve risk management issues according to policy or protocol; Effective time management; Ability to work as a team member and autonomously; Good interpersonal skills. Desirable: Able to manage, monitor and review long term conditions; Knowledge of QOF, enhanced services, and primary care targets; Strong IT skills; Competent in use of SystmOne; Competent in the use of Office and Outlook; Audit and research skills; Teaching and mentoring skills.
