We are looking for a dynamic, outgoing and caring individual who likes a challenge to join our team of experienced Care Managers to lead a small team of Support Workers providing them with line management support and supervision. To implement and contribute to a Person-Centred Approach through thorough care planning. To Safely support and promote the independence and choice of individuals within their own homes whilst respecting and encouraging a culture of equality and diversity. To ensure the People we Support are actively involved within their community whilst keeping them safe from abuse or harm.
