We are looking for individuals who are passionate about working with people who have learning disabilities and their families. If you are committed to supporting people with learning disabilities to live autonomous lives and achieve positive wellbeing, then we want to hear from you. This is an exciting opportunity to be based within a local authority integrated health and social care service. The service comprises many disciplines including Social Work, Community Nursing, Psychology, Occupational Therapy, Speech and Language Therapy, Art Therapy, Physiotherapy and Psychiatry. The Care Coordinator post will sit within the social work team.

Key Responsibilities

  • Provide face‑to‑face support to service users, their families and external partners.
  • Identify whether support packages continue to meet the needs of service users and their families and make recommendations regarding support packages.
  • Arrange and amend support to ensure needs are being met in a person‑centred and strength‑based way.
  • Support integrated front‑door duty service on a rota basis, resolving crisis situations.
  • Undertake assessment work and safeguarding case work.
  • Attend multi‑disciplinary meetings to present cases, communicate effectively with service users, families and internal/external colleagues and partners.
  • Write accurate, comprehensive support plans and assessments.

You will need excellent communication skills, both verbal and written.
