We are looking for compassionate, reliable and professional Support Workers to assist adults within supported-living services.
You will help the people we support live as independently as possible, make their own choices and participate in their local community. Support must be delivered with dignity, respect and a person-centred approach.
Key Responsibilities
- Supporting individuals with daily routines and independent-living skills
- Providing personal care where required
- Supporting medication in accordance with care plans
- Helping with meal preparation, shopping and household tasks
- Supporting appointments and community activities
- Promoting choice, independence and social inclusion
- Following individual support plans and risk assessments
- Maintaining clear and accurate records
- Reporting concerns and safeguarding the people you support
- Working professionally with families, colleagues and other professionals
