Want a role where you can make a real difference to people's lives?

At FUSION HOUSING, we support people across West Yorkshire to find and maintain a safe

and suitable home, access opportunities and build a more independent future.

We're looking for a compassionate, motivated and approachable Support Worker to join our

HOMEs team in Kirklees.

You'll work closely with adults with a range of support needs, helping them build confidence,

overcome barriers and work towards their individual goals. At the heart of the role is building

positive, professional relationships and empowering people to move towards greater

independence.

No two days will be the same, so we're looking for someone who enjoys working with people,

can think on their feet and wants a role where they can see the difference their support

makes.

What you'll be doing

As a Support Worker, you'll get to know the people you support and work alongside them to

identify what's important to them and what they want to achieve.

Your role will include:

Building positive and professional relationships with clients and helping them work

towards their individual goals.

Monitor client progress against agreed support goals, celebrating achievements and

adapting support approaches to meet changing needs.

Support clients to sustain their tenancy, manage their accommodation responsibilities,

and develop independent living skills such as budgeting, meal preparation, accessing

benefits, and maintaining a safe home environment.

Supporting individuals to build their confidence, independence and relationships with

others.

Provide person-centred, trauma-informed support to adults with a wide range of

support needs, including mental health conditions, learning disabilities, neurodiversity,

substance misuse, physical health conditions, and other complex or multiple needs.

Build positive, professional relationships with clients, offering practical and emotional

support during difficult periods while empowering individuals to develop the skills,

resilience, and confidence needed to achieve their goals and maintain independence.

Work collaboratively with Housing Workers, Social Workers, drug and alcohol services,

healthcare providers, and other partner agencies to coordinate support, advocate for

clients, and promote positive outcomes.

Complete, review, and maintain accurate support plans, risk assessments,

safeguarding records, and case notes, ensuring records are timely, outcome-focused,

and compliant with organisational policies and data protection requirements.

Identify and respond appropriately to safeguarding concerns, wellbeing issues, and

risks, taking prompt action in line with organisational procedures and statutory

guidance.

This is a Monday-to-Friday role with no weekend working, offering a great opportunity for

someone looking to build a career in support while maintaining a regular working pattern.

You'll be someone who:

Is compassionate, approachable and genuinely interested in supporting people.

Can listen without judgement and build positive professional relationships.

Has experience of supporting or working with adults who may be vulnerable or have

additional needs.

Can remain calm and professional when faced with challenging situations.

Is positive, enthusiastic and curious, with a willingness to find creative solutions.

Enjoys being part of a supportive, fast-paced team where everyone is willing to get

stuck in.

Is organised and comfortable managing a varied and changing workload.

Can use their own initiative while knowing when to seek support or advice.

Is committed to providing person-centred support that enables people to become more

independent.

Your experience doesn't necessarily have to come from a housing setting. You might

currently work in social care, mental health, homelessness services, community support or

another people-focused environment and be looking for your next opportunity. We're

interested in who you are as much as the experience you bring.

You'll also need to:

Hold a full UK driving licence and have access to a vehicle that you can use for work, as you'll

need to travel between our HOMEs properties in Huddersfield and Dewsbury to visit and

support clients.

Why work for Fusion Housing?

As well as a rewarding role where you can make a genuine difference, you'll receive a great

range of employee benefits, including:

30 days annual leave

2 additional floating days

Statutory bank holidays

30-minute paid break each day

Flexible working
