Job Overview: This role supports the delivery of high‑quality patient care by providing specialist pharmacist support within the multi‑disciplinary anticoagulation team. The post holder will work closely with colleagues in the team and across the hospital and community to ensure a safe and efficient anticoagulation service.
Key Responsibilities:
- Provide expert anticoagulation advice to clinicians and patients.
- Ensure anticoagulants are used appropriately.
- Overall responsibility for organising and delivering anticoagulation and VTE training across the Trust.
- Ensure anticoagulation and VTE prevention guidelines are in place and regularly reviewed, updated and accessible; work with clinicians to develop necessary guidelines.
- Participate in relevant meetings and actively engage in service development, including undertaking regular audits.
- Work collaboratively with the whole anticoagulation team and colleagues across the Trust in a friendly and professional manner, and mentor and support the Band 7 Anticoagulation Pharmacist.
- The post holder will manage their own workload, prioritise tasks effectively and meet deadlines, working independently as well as part of a team.
- The role requires great attention to detail, excellent communication and organisational skills, and the capacity to make high‑level decisions about anticoagulation treatment.
- The post holder will work on site at Princess Alexandra Hospital Monday‑Friday based with the Anticoagulation team in Galen House.
