Mayflower Medical Group is looking to recruit an ACP/ANP to join our clinical team for 28.5 hours a week, working across 3 days.

The Advanced ClinicalPractitioner (ACP) plays a pivotal role in enhancing the level of care providedto patients through advanced practice interventions. This role involvesautonomously managing complete episodes of clinical care across all age groupsand all demographics, encompassing a wide range of clinical conditions,diagnosing, treating, and referring patients as appropriate. The ACP operateswithin a multi-disciplinary team and is instrumental in mentoring andsupporting both nursing staff and trainee practitioners.

Key responsibilities includethe application of advanced clinical skills to assess, diagnose, treat, andadvise patients across a variety of settings including face to face, telephonevideo and econsultations, ensuring the use of evidence-based practice. The ACPwill also lead in the development of new services and pathways to improve thequality of care and patient outcomes. This role requires dynamic clinicalleadership and active participation in practice research activities to promotea culture of learning and continuous improvement within the practice.

Main duties of the job

Responsibilities:

Patient Care:Deliver advanced and complex clinical care, providing assessment, diagnosis,treatment, and prescribing for patients with acute, chronic, and complexconditions.

AutonomousPractice: Work independently within the scope of practice to manage completeclinical episodes, using advanced diagnostic skills and clinical judgment.

Case Management:Manage patient cases from initial assessment to conclusion, includingfollow-ups and referrals, ensuring continuity of care.

Prescribing:Utilise prescriptive authority responsibly following current guidelines andprotocols to enhance therapeutic outcomes for patients.

Home visiting:undertake home visiting as and when required, including triage of home visits.

Mentorship:Provide mentorship and supervision to nursing staff, trainee advancedpractitioners, and other healthcare professionals to enhance team competencies.

ServiceDevelopment: Lead the development and implementation of new clinical servicesand pathways, improving processes and patient care.

Professional Development:Engage in continuous professional development to keep abreast of medicaladvancements and to further develop clinical skills.

Record Keeping:Maintain comprehensive and accurate patient records using SystmOne.

Meetings: Attendand contribute to team meetings, providing updates and insights on patient carestrategies and outcomes

The Fuller and Forbes Partnership has recently acquired the Mayflower Medical Group, the wider organisation has practices currently in Leeds, Devon, Burnley and Gateshead, providing care to over 120,000 patients.

The Mayflower group, based in Plymouth provides care to 35,000 patients across 5 locations; Stirling Road, Mannamead, Trelawny, Mount Gould and Ernesettle. Registration withthe Nursing and Midwifery Council (NMC) or Health and Care Professions Council(HCPC) or other regulatory body as appropriate.

Masters degreein advanced clinical practice, or active and up-to-date accreditation throughthe transition route, or any nurse, paramedic or clinical pharmacist that hassuccessfully completed an ePortfolio (supported) route and has received theirdigital badge of recognition and has continued to engage in ongoingprofessional development to maintain their status .

Independent andsupplementary prescribing qualification.

Clinical Skills:

Extensiveexperience in managing complete episodes of clinical care including assessment,diagnosis, treatments, and prescribing.

Advanced clinicalskills in a primary care setting, with proficiency in using clinical diagnostictools and conducting complex clinical procedures.
