The Role:
You will be responsible for contributing to the holistic assessment, planning, delivery and evaluation of patient care.
On a Day-to-day Basis, You'll Be:
- Providing 45-minute (minimum) patient visits
- Offering personal and clinical care, including IV therapy, and wound management
- Monitoring and recording conditions and escalating cases when needed
Whether you're initiating health promotion activities or liaising with other members of our multi-disciplinary team, you'll help us to empower our patients in their choice to be treated at home.
